  • Disinfection: 55% of participants use hydrogen peroxide (Clear Care, Clear Care Plus or 3% H2O2) for daily disinfection, while the remainder were split fairly evenly among the multi-purpose solutions Boston Simplus (15%), Tangible Clean (13%) and Unique pH (13%).
  • Rub cleaning: Only 65% of participants rub their lenses clean on a daily basis.
    • Among those who do, 65% rub before disinfection, 31% after disinfection and 4% both.
    • Boston Simplus was the most common solution used for rub cleaning, followed by Unique pH and Tangible Clean - but 27% of "rubbers" are using either an alcohol-based cleaner no longer in production or another solution not labeled for this use.
    • Among those who do not rub, the top reason cited was that it is not part of their provider's instructions.
  • Extra enzyme cleaners: Use of these products is uncommon; only 3% of participants use this type of product at least monthly.

  • Rubbing before vs after disinfection: The 30% of "rubbers" who said they rub after rather than before lens disinfection are *probably* mostly the same 30% who said they use Boston Simplus. Boston Simplus' instructions specifically say to rub after disinfection, while the other multi-purpose solutions say to rub before. This is unfortunate. Removing surface deposits from scleral lenses makes most sense for ensuring that they get completely sterilized while soaking. 
  • Terminology confusion: This is a longstanding problem, and the survey results were no exception. From comments, it was clear many people are using their words differently. I want to post the following suggestions for how to understand the terms.
    • (RUB) CLEANING: This is a process to remove surface deposits. It is done with a solution that contains surfactants, either a multi-purpose solution or a cleaning solution. Multi-purpose solutions can be used for BOTH cleaning and disinfection. Cleaning solutions can only be used to clean, not to disinfect. Cleaning is not a substitute for disinfection. Salines (whether preserved or not) do not clean lenses.
    • DISINFECTION: This is a daily process that sterilizes the lenses and takes several hours. It should be done with a solution that has been proven effective specifically for rigid lenses. These include Clear Care (both types). Boston Simplus, Unique pH and Tangible Clean.
    • PROTEIN REMOVAL: Most cleaning and disinfection solutions claim to remove proteins, but there are also specialty products designed to address this specifically.

  • Every six days I use one of the Clear Care products for the overnight disinfection/cleaning. 
  • Despite dutiful night cleaning and disinfection, I find small cloudy protein deposits which I am unable to clear. [Have you tried Progent? How old are your lenses, and has your provider examined them recently?]
  • I wish doctors offices would give more instruction about this. Mine sent me home with clear care with Hydraglide and a bottle of Biotrue, but no instruction on what to do with the Biotrue. Plus, the Biotrue wasn’t listed on the paperwork they gave me about how I should clean it. Their office staff even referred to it as “saline”, which I knew that it wasn’t. But that seemed like a dangerous comment to someone who might’ve thought that meant they could fill their lens with it. Also, since it wasn’t listed on the paperwork as approved for use with Hydrapeg coating and I knew that I had that, I felt uncomfortable using it, especially considering how little instruction there was overall. This little bio true episode has been my most frustrating thing with my doctor’s office about scleral  lenses to date. [Sorry to hear that. I have a suggestion: Could you let your provider know about the Dry Eye Foundation’s handbook for scleral lens users?]
  • What happened to the new clear care containers? They’re harder to close when you can’t see. [DEF is investigating some issues with the new cases and is in communication with Alcon about them. We encourage reporting all issues to Medwatch.]
  • I do minimal cleaning and have never had a problem with build-ups. My lenses last 5 years or more.

  • CLEAR CARE + is a peroxide cleaner. I do not use enzymatic cleaners.
  • I have had scleral lenses for a little over a month and so far all is good with the process I am doing.  Inserting is getting better.

  • Products checked have been used over 5 years on my PROSE lenses.

I was also told that if I use different products, it might eat away the coating, so beware! Admittedly I do sometimes use the store brand versions of the Clear Care Plus to save money.
  • I remove and disinfect/clean my lenses daily. However, I occasionally forget and sleep in my lenses. When that happens, I remove my lenses, rinse with saline, and re-insert. 
  • I only use what was exactly told by doctor. Wish there was a book or a way to know which solutions in the market can be used. [There is - see the MBFSL Patient Handbook from Dry Eye Foundation, also see Cleaning & Care section at mbfsl.org.] For example, Costco had a sale for BioTrue and my parents wanted to get it for me as a gift. But I had no idea if I could use it or not. My doctor only ever told me OptiFree, Clear Care, ScleralFil, and Purilens.

  • I would like to understand how to "rub" clean better. My daughter has profound autism and I have to take care of her contacts for her as she is not cognitively capable. When they get build up, she gets very frustrated and complains about them being foggy. I can't afford to risk hurting the lenses with a cleaner I've never used. So I resort scraping them with my fingernails and trying to get the buildup off. Cleaning products are scary and the choices are overwhelming! My eye doctor is very nice, but he doesn't now much.  [Give us a call or come to the Dry Eye Foundation’s monthly Zoom group for scleral lens users - love to help.]
  • I always wash my hands with soap and water before using Boston Simplus and in the morning before using Purilens

  • My provider originally never told me I had to use a cleaner :(. For the first week or two of using sclerals I stored them in Purilens. Very bad! If you did not receive written instructions from your provider I would encourage you to suggest that they reach out to Dry Eye Foundation for a free sample of the patient handbook, practices can purchase them for their patients.]
  • The prose disinfecting threads break too easily  when closing the lid. [That’s interesting, I haven’t had that happen personally. It’s important not to over-tighten them though as the case itself can crack.]
  • I soak in Unique ph one X per week. Haven’t yet done a progeny deep clean [I’m curious what you do for the rest of the days, what do you soak them in? Lenses have to be disinfected every day.]

  • I use Clear Care Plus Hydraglyde daily. I use progent x2 a month. This routine is the best way I have found to keep my lenses clean and free of deposits. It has also extended the life of my lenses. I only use the Unique PH and Boston Simplus after a progent treatment. In the past these 2 products on their own have not worked well for me in keeping my lenses clean.

Have tried many cleaners and Tangible clean and the way I care for my lenses allowed the 1st set of Eye Print lenses to last for over 5 years with just light scratches.   • I was using clearcare every day until I learned that it was causing fogging for me. Now I only use it once per week.  [Was it Clear Care Triple Action or Clear Care Plus? Some of us report worse results with the Plus version.] I also use the Walmart brand and have not noticed a difference between that and Boston Simplus.
  • It’s been awful with the smudge questing in the outer legs each eye. Never an issue in 10 years of wearing sclerals. Now more than daily. [According to the survey, the community is split right down the middle on Triple Action versus Plus/Hydraglyde.]
  • Rubbing with my finger isn't enough -- I have to use a q-tip or sponge to get the protein off. When I was only using my finger, I had to use Progent every 5 days. Daily rubbing with a Q-tip or sponge allows me to reduce Progent to every 2 weeks. This was a tip I got from the FB group.
  • I use Sereine extra strength daily cleaner nightly before sanitizing 
  • "I’ve found it works well to quickly clean or submerse my lenses in unique ph then to insert the lense after cleaning in hydraglide overnight.
  • I use Sensitive Eyes Saline Solution to rub clean scleral lenses before disinfecting. [That is not a cleaning solution. It’s just a saline, so it’s fine for rinsing.]
  • I simply use Boston cleaner to rub before putting it in Boston conditioner overnight. 
  • I have not found any discernible difference between ClearCare and ClearCare Plus, although my eye doctor prefers ClearCare. They both do the job wonderfully. I use whichever one I can find at my local Costco. When traveling, I use Unique pH because it's more convenient and easier to travel with and deal with.
  • "I am on the waiting list for Tangible Boost. I'm hoping that will help. I do struggle with protein buildup. I find I have to use ""Refresh Relieva for Contacts"" or ScleraFil several times during the day to rewet my eye. (I'm on PredMild which increases my dry eye symptoms.) 
  • I have completely cut out any aerosol or pressurized spray product. It doesn't matter how careful I am, when I use these products, I have to take my contacts out in early evening due to discomfort.
  • I use the Purilens for rinsing after I use the Tangible Clean, then put the lenses in ClearCare Plus for overnight.  Inserting the lenses has been a long, frustrating learning curve. I've found I need the ScleralFil to insert the lenses. The ScleralFil is just enough thicker it works better.
  • I've worn various contact lenses since 1969. I was one of the participants who participated in research for ""breathable lenses."" So, I'm a very experienced user. I moved to very large (17.5) EyePrint lenses in October. They have been a real challenge for cleaning, inserting, wearing, etc. I am grateful to have them because my vision has significantly improved, but at times they are a test of my good humor."

  • I normally ask my provider to clean my lenses thoroughly. However, seeing the Hydrogen Peroxide 3% method in the listing, I might give it a try.

Occasionally I use a small drop of dawn dish liquid [not recommended!] and gently rub each side then rinse thoroughly with Purilens then overnight disinfectant with clear care. Also my question? I disinfect my lens every night in clear care is this correct? [YES that’s perfect] Thank you
  • I use the sodium chloride when my contacts fog or I cry just to clean them. [I think you mean rinse?]
  • My doctor told me for quick disinfection to soak the lenses in hydrogen peroxide or Clear Care (advises against Hydroglide) for 20 minutes. Then remove, rinse and place in fresh saline for 5 minutes, take out and rinse, get new saline and soak for another 5 minutes. Then they’re ready to go. I’ve needed to use this in place of the standard overnight on a few occasions.  Seems to work great! [Technically, complete disinfection takes 3 hours at full strength hydrogen peroxide, so this is not a substitute, but it’s probably what I would do too if necessary in a pinch.]

  • "1. Clear Care with Hydraglide stripped the Hydrapeg coating from my lenses.  I had to purchase a new pair(!!!) [1) Clear Care Plus is officially compatible with HydraPEG so I don’t feel this is correct. For some of us, the coating wears off faster than for others, and of course rub cleaning will also wear it down faster. 2) Stripping the HydraPEG does not ruin a lens. However, if the lab is not willing to re-coat it, and if you can’t wear it without the coating, then unfortunately replacement can be necessary.]

[Yes, but it will matter more for some patients than others.]  use clear care, should I also incorporate enzyme cleaning? [Additional cleaners are an ‘optional’ if your deposits don’t come off with ordinary care.] Or does the two do the same thing? [Most products claim to remove protein deposits but some do it more effectively than others.] Can Dry Eyes do/commision some studies with microscopes, etc to see what actually works better? Thanks! [Technically, they all “work” - they have to prove that to the FDA - but then how does it play out in practice for patients. Scleral lens users have a wide spectrum of diseases and we have different needs. This is why we run surveys :) We’ll continue to….]
  • I use biotrue for nightly disinfection, rub clean in the morning with purilens plus and use the same solution for daily wearing. [Biotrue is not approved for gas perm/scleral lenses.]
  • Per the scleral lens' manufacturer I rub my lens before soaking daily in Boston Simplus. Additionally, the manufacturer says to use Clear Care once every month. Since Clear Care can only be used for ninety days, I use it once a week until the eleventh week. Then, I use it every day until it is empty thus preventing waste of the Clear Care. One month later I restart the cycle over. [Interesting!]

  • When I first got my PROSE lens, I was told to use Miraflow.  I ordered more maybe twice, because I use only 2 drops per application (applied directly after removing the lens), and it therefore lasts a long time.  I order it directly from the maker.  After use, I rinse with saline before a soak.  Note that when I travel, I only use the Miraflow (no Clear Care disinfectant) and that seems to be fine.  I need no other cleaning, and the clinicians are always commenting on how well maintained my lenses are.  I see that the stuff is currently not available (MIRAFLOW.com) but I have 1 bottle in use and another backup so I am good for years. [I couldn’t understand from what you wrote whether you are disinfecting your lenses. It’s very important that they get sterilized between uses. Surface cleaning is different from disinfection.]

  • I am very sensitive to bio true and quite sensitive to unique Ph. If my lenses are disinfected in either solution they irritate my eyes

  • I use Unique ph (for manual cleaning and disinfection) when traveling, otherwise I use Boston Simplus and Clear Care.  All products seem to work great.  
  • Have Hydra-peg coating. Have never been given specific instructions other than to use Boston Simplus 
  • I have found it helpful to occasionally use Serine which is a soapy cleaner 2-3 times per month for a deeper clean. I have not needed to use a product for protein as I don’t seem to build up much protein on my lenses

  • After removing lenses from clear care, I rinse with bio true. Then rinse with preservative free saline. [Not clear to me why you would add the biotrue step. If it’s to get the peroxide off - assuming you’re using ClearCare correctly, the peroxide will have been completely neutralized by the time you take the lenses out, so a quick rinse with PF saline should be all it needs.]

  • Should I see some protein build up on my lenses after soaking all night long I use my plunger to gentle rub it off. I stopped rubbing my lenses after my provider told me my lenses were scratched, I couldn't understand how this could be unless my rough fingers from quilting were the reason. I now put my lenses in a large case provided by tangible clean and swish them around in the cleaner after removal and before inserting hoping this will aide in the cleaning process. [There are pads you can get that are safe for rubbing lenses clean if for any reason you can’t use your fingers.]
  • Became sensitive to Boston Simplus due to the preservative. Just use saline preservative free solution to clean [I think you mean rinse] and rub and clear care every night [Again, clear care great, but saline does not clean lenses.]
  • My original provider instructed me to rub my lenses and never use a peroxide cleaner.  My current provider switched me to ClearCare in 2019 and advised me not to rub my lenses, but if I ever did I may use a Q-tip brand cotton swab only.  (Other brands may have synthetic fibers that could scratch the lens.) [Quite right re cotton.]

  • Many one offs on cleaning.  Depends if changed/rinsed lenses and how many times during the day, depends if in more dust or outside during the day, stress. Depends if I spray them before I put them in the cleaning solution for the night. [Spray?]

  • Mine have hydra-PEG coating, and I'm unsure what is compatible.   • I use Clean & Clear Hydraglide to occasionally enzyme clear.
  • I miss Lobob.  It felt like it did a good job cleaning, even mid-day with mucus buildup.

  •  I use clean and clear [I assume you mean Clear Care] regularly except when needing to travel. I only have scleral in 1 eye, other has an Ahmed valve so not considered a candidate though would be greater if I could. 
  • There was not a place to put ‘other’ rubbish cleaner. I use biotrue MPS to rinse off simplus and rub clean. [Of the two, only Simplus is approved for this type of lens.]

  • I had a bad bout of blepharitis for 18 months, when my lens crusted up badly during the day and could hardly be removed.  It gradually improved with this cleaning process at bedtime:  rub lens with Unique pH until clean, rub with Scleralfil until squeaky, soak overnight in Boston Simplus.  In the morning, I need to use Unique pH again to rub off the slimy Boston solution, then rinse well with Scleralfil before inserting.  My eye doesn't do well with any of the hydrating drops or other solutions touching it.  Once a month, I do soak the lens in Clear Care and then use Tangible Boost, to repair the hydra-peg coating.  (Progent actually stripped the coating!) [Progent is not compatible with HydraPEG.]
  • When traveling, I use Unique pH instead of Clear Care Triple Action because of the smaller bottle (2.5 oz.).

  • "After replacing my scleral lens twice using clear care & progent my provider changed my routine to Boston simplus only 2 years ago. I also used Tyrvaya nasal spray until this year but stopped due to cost 
  • I still get a build up of mucous across the center of my lens by mid-day.  It is frustrating!  I either take it out and clean it before reinserting, or just leave it out.  Then, my eye has a scratchy feeling.
  • Thank you so much for looking into this issue. I have been having more problems than usual lately with the clouding of my left sclera lens, (which is the eye that has severe dry eye) and I've been wearing sclerals for over 10 years now. My optometrist doesn't seem to understand that the last replacement lens I received became cloudy within a month! I've been using Tangible since when it first came out with good results, although after about a year it tends to get hard to control the clouding. I use a gel at night and know this and my inability to blink completely on this eye are at the root of my problems but I'm not getting the help I need from my eye care department at Kaiser.
